The longer the posting is, the bigger it is, and if you fail any of
these big posting, you will need to do remedial at the end of yr 4.. !!!
- preparation ?
make your own notes (if
can, one book each posting, depends on you..) for every posting. You can
put it in something like how to take a history generally and for
different cases, physical examination for that particular posting, topic
of diseases according to the books or log books, and other things like
teaching from specialists, important notes on certain things like
immunization schedule, growth chart and so on. I know you have a lot of
reference books and notes from seniors, which is why it's important that
you put everything into just one book of yours, yes it takes time but
it will come in handy when you reach year 5. You will have time to do it
before and during even after the posting, don't worry. And you have
such a long holiday every end of year isn't?
- first posting
each the diagnosis on your own, think of the differential diagnoses and
rule them out one by one in your history, do your PE, think of the
investigation with the expected result and have in your mind what
management should be given to the patients, then only you look at the
patient's folder. Compare your notes with doctor's notes and see how you
can improve yourselves, and you can even discuss with the doctors.
you should know everything about them - current management, and you should follow up
them everyday until they are discharged.. You will also need to cover
patients before the morning round everyday (around 8am), so it's advised
that you clerk your patients the night before that .
PROCEDURES
taking blood, setting IV line, you can learn how to prepare nebuliser, present cases to the MO, do discussion with your seniors and friends, filling up log books and so on .
CLINICAL TEACHING
- the subgroup leader will need to assign someone to present the case
(better if everyone gets their turn to present),
- contact the particular doctors (also stated
on time table) at least 2 or 3 days before that teaching. The teaching
can be held in the ward or department, depend on the doctor.
-During teaching, make
sure you all be punctual ok, you don't want to irritate the doctors (who will be your examiners some day).
-should read about the topic of the case that will be presented. Doctors love to shoot questions you see.
SEMINARS
* you should be able to talk and explain your topics now, not to read from the slides..
-in tutorial
ONCALL
*on call according to the time table. Nothing scary, you will do it in groups like once or twice in a week, and you just need to go to the wards like you have done everyday, just that you need to do it when everyone else rests in their hostels.. You will need to be in the wards before 8pm (not for all posting), clerk all the new patients and the patients in acute cubicle (basically, the patients whom the on-call doctors will see that night), the doctors will usually come at 8 or 9pm. You may need to call the MOs who will on call that night, tell them you will be on call that night and ask them are there any new patients for you to clerk, or where and when will the round starts. When they start the round with the specialists, make sure you follow them, offer to present the case or you can just stay silent, ask questions, discuss with the doctors, let them shoot (and teach) you, and finally let them sign your attendance (bring a name list with you!).
clinics
*all the posting (according to the time table, or whenever you have free time). Be nice with the nurses there, ask them where should you put your bags, then either you clerk the patients sitting outside doctor's room and follow them into doctor's room when their number or name is called, or you can straight away go into the doctors' room to see the patients together with doctors .

clinical year is not like the years before...
u have to plan ur own study schedule...
no one will spoon feed u...
so... better plan what u wan to cover and study for every weeks....
and better form a study group to discuss and share wat u all learn in the wad, clinic and teaching...
before enter clinical year.... basic must be good....
if u r very hardworking... try finished kumar& clark or davidson..... (i not count harrison cuz i know u all not able finished it within a holiday)
If u r moderate hardworking, finished oxford handbook of medicine...
If u r a bit hardworking, just cover important diseases in those books or cover all the previous notes.
If lazy o...... then just pick some important disease lecture notes to study lo
